Job Title: Therapist Position Number: 041268 Department / Hospital: PEDIATRICS: NEONATOLOGY BANDSTRA The University of Miami, Miller School of Medicine is currently seeking a Therapist to work for the Department of Pediatrics. In this capacity, the incumbent will provide maternal child health education and counseling, parenting, infant mental health and developmental interventions, and other psychosocial and referral services to mothers/infants and their families participating in the Jasmine Project, a Federal Healthy Start Project funded by HRSA to reduce perinatal health disparities and improving birth outcomes among Black infants in Miami-Dade County. Conduct individual and group health promotion education and counseling activities to pregnant and inter-conception women including: prenatal education, childbirth education, inter-conception behavioral health care, infant health care, and breastfeeding education and support. Conduct individual and group parenting and infant developmental interventions and counseling services, including: infant mental health/dyadic therapy, infant developmental screening assessments, developmental consultation and guidance to parents, and lead established programmatic group and individual parenting curriculums. The minimum qualifications for this position are as follows: Bachelor’s degree from an accredited university/college with a concentration in Social Work is required. Master’s degree from an accredited university with a concentration in Social Work is preferred. State of Florida licensure in the capacity of Social Worker is preferred. Experience in maternal and infant health and knowledge of maternal-child health issues, infant mental health, parenting and child development issues, counseling and social work skills, and knowledge of mental health and substance abuse issues in the target population is required. Position requires exceptional skills in oral/written English; experience working with mothers/infants/families; provision of services through home visitation and ability to work occasional weekend and evening hours. Any appropriate combination of education, certifications, and/or relevant work experience will be considered.
Job Title: Senior Case Worker Position Number: 041270 Department / Hospital: PEDIATRICS: NEONATOLOGY BANDSTRA
The University of Miami Miller School of Medicine has an opportunity for a Senior Case Worker within Pediatrics / Neonatology, located on our Medical Campus. This individual will provide maternal child health care coordination / case management and wrap-around and referral services to mothers/infants and their families participating in a Healthy Start Project focused on reducing infant mortality and improving birth outcomes among Black infants in Miami-Dade County. Candidate will conduct home visits in Miami-Dade County. Should have community service referral skills, to provide case management and care coordination services focused on maternal child health issues to pregnant women, mothers/infants and their families, including family service planning, prenatal care and medical home/primary care linkages and follow-up, referrals for WIC, Medicaid, and other psychosocial, mental health and substance abuse treatment referrals as needed, working closely with other community agencies to ensure successful referrals and non-duplicated service delivery. Provides individual and group Healthy Start wraparound services in the home and clinic settings including: health promotion education, breastfeeding support, tobacco cessation, conducts Ages and Stages Developmental Assessments and parenting education activities to pregnant and interconception women.
Bachelor's degree and three years minimum of experience regarding maternal-child health issues, parenting and infant development, and knowledge of mental health and substance abuse issues in the target population. Position also requires exceptional skills in English, oral and written, as well as, provision of services through home visitation and ability to work occasional weekend and evening hours. Job Title: Case Manager II/Care Coordinator II
Essential Functions: ESSENTIAL FUNCTIONS MAY INCLUDE:
• Provide case management services (information, plan referral and linkages). • Develop and maintain client records and program documentation according to Healthy Start contract and CHS standards (assessments, treatment plans, progress notes, termination summaries, etc.). • Conduct home visits with teen moms, pregnant moms and new moms and their families. • Conduct one on one education with clients within the home. • Enter client data into database. • Conduct written communication with client and providers. • Prepare and mail educational packets to clients. • Facilitate educational classes such as parenting skills, breastfeeding, prenatal classes, etc. • Coordinate services with other professionals and para-professionals. • Conduct community outreach as needed. • Coordinate and participate in community outreach events. • Participate in community and staff meetings. • Attend trainings and conferences • Arrange for transportation of clients to classes. • Prepare individualized care plans and family support plans with clients. • Participate in the quality improvement process.

PREFERRED: • Previous experience working with at-risk children and families. Previous experience conducting home visits. Bilingual in English/Spanish or English/Creole preferred, but not required.
