Healthy Start Coalition of Miami-Dade Outreach Initiative

Formerly known as the Liberty City Outreach Initiative, the Healthy Start Coalition of Miami-Dade Outreach Initiative is  zip code targeted in the Liberty City area of Miami-Dade County.

Funded by the United Way Success By 6 from October 2002 through September 2005, this  Initiative has been spear-headed by the Healthy Start Coalition of Miami-Dade with project partners, Economic Opportunity Family Health Center (EOFHC), Teen Pregnancy Prevention Center (TPPC), and the Health Council of South Florida (Council).  The Outreach Initiative targeted zipcodes 33147 and 33142.  Over the three year period, the Coalition identified the need to provide outreach in additional high risk neighborhoods in Liberty City.

Through funds provided by the Department of Health, the Coalition continued with this outreach effort.  Currently there are nine zipcodes in which outreach is conducted. Those zip codes include 33142, 33147, 33167, 33150, 33127, 33169, 33054, 33055, and 33056.

The goal of the Initiative is to increase the Healthy Start Prenatal Screening rate and subsequent enrollment in the Healthy Start program among residents of the targeted zip codes. The long-term goal and primary impact of the Initiative is to reduce the rate of preterm births in the targeted zip codes.
