Closing the Gap
Interconceptional Education Outreach Project

Funded by the Florida Department of Health, this project provides comprehensive information, education, and support related to the optimal health status needed by minority women of reproductive age to improve the birth outcome of a potential pregnancy. The program promotes health awareness through community outreach,
reduces risk factors for poor birth outcomes through referrals and education classes, increases community collaboration and access to care, and promotes client participation in local health program planning.

These services may include information on:

  • access to care
  • baby spacing
  • nutrition & physical activity
  • chronic health problems
  • substance abuse & smoking
  • mental health
  • environmental risk factors

Eligible clients are African American, Haitian or Hispanic women who are not Healthy Start program participants but who have been identified as being at risk for poor birth outcomes.
