What is the purpose of MomCare?
The program seeks to improve birth outcomes and infant health by providing Medicaid-eligible pregnant women with:
1) Simplified Medicaid enrollment
2) Choice Counseling
- To help women understand their choices and select a primary care provider
To assist pregnant women in receiving early prenatal care including scheduling and keeping appointments, following through on referrals for other services and identifying and resolving client problems
3) Case Management
Case Management identifies at-risk women through the Healthy Start screening process and assists at-risk women in obtaining needed services. Case management is an on-going process to track a client and identify client needs and assist her in accessing services
Other goals of MomCare include:
- Increase Healthy Start screening rates
- Increase participation in the WIC program
- Assign a primary care provider within 30 days
- Identify any client needs
- Increase family planning services
- Ensure health insurance for the newborn
MomCare Federal Requirements:
- We do not recommend any particular provider
- To avoid recommendation we talk about what might be considered in making the choice (location, hospital, office hours, etc.) and provide specific characteristics of each provider to help her make a choice.
